KMST collects the following data:

· Health and medical related information that are relevant and necessary to your physiotherapy and scar therapy.
· Details of other health and social care professionals that you may be involved with.
How do we collect your data:
· We collect data regarding your name and contact details when you make a telephone or email enquiry or book a physiotherapy appointment.
· We collect personal health and medical information and details of other professionals involved at the time of your initial physiotherapy consultation and as relevant at the time of any further follow up appointments.
· KMST may also receive your data indirectly from a referrer or other health or social care professional only if you have already provided consent for this to occur.
How will we use your data:
KMST collects your data so that we have an accurate legal record of our interventions with you in line with the health and care professions council (HCPC) standards of conduct, performance and ethics.
We will only share your data with other health and social care professions if you have agreed and consented to this only for the purpose of managing your health.
We may also use your data to:
· Respond to enquiries
· Manage appointments and treatment
· Process payments
· Improve our website and services
We will not share your data in any other circumstance.
How do we store your data?
KMST securely stores your health and medical related data electronically using encrypted and password protected software. Occasionally we may be required to hold a small amount of paper-based information which is stored within a locked cabinet in a locked room.
KMST securely stores your contact details electronically using encrypted and password protected software.
We store your data for 8 years in line with HCPC legal requirements for health-related records. Once this time has expired, your data will be destroyed securely.
